Midwest Central School District #191 will conduct a free
screening Tuesday, May 4 from 2:00 - 6:00 PM, for children ages
3 - 5 years old who reside in the Midwest Central School District for
the 2023-24 school year. Your student must turn 3 by September 1.
The screening will be held at the Primary School located at
450 Southmoor St / Manito, IL 61546
Children will be screened by appointment only
You will need to bring the following documents to complete the preschool screening and registration;
Certified Birth Certificate (not the one from the hospital)
Current Physical and Shot Records
Proof of Residency
Photo I.D.
Income Verification (2 consecutive recent pay stubs, 2022 W’2’s, Letter of employment verification, SNAP, WIC, TANF)
